Home Help through MDHHS where a family member or friend is the paid caregiver and enrolls in CHAMPS.

For the family member or friend who will be paid to give the care: their application, and the details for their MSA-204 CHAMPS enrollment form.

The client or their family pays directly, with or without long-term care insurance behind it.

Follows the private pay packet: the Service Agreement signed, plus card or bank details for weekly invoicing.
